About Upper Hume Primary Care Partnership (UHPCP)

Our Vision

The people of the Shires of Indigo and Towong, the City of Wodonga and the Kiewa valley
are proactive and inclusive in achieving and sustaining health and well being

Our Mission

We will work with our people to identify their health and well-being needs as they live through their life stages. 
We will plan collaboratively for their future and partner across our community, services and sectors to identify strategies that meet these needs.

Our Key Objectives

Our key objectives are focussed around four areas of work:

  • Parternship
  • Integrated Health Promotion
  • Service Coordination - including e-referral
  • Integrated Chronic Disease Management

How we Operate

The UHPCP is structured around a General Committee.  This committee comprises two levels of membership, full members and affiliate members that are signatories to the UHPCP Memorandum of Understanding.  The General Committee is responsible for setting the direction of the PCP, decision-making, and ratification of the directions recommended by working groups and platforms.  The General Committee is supported by a Planning Group (chair-people of platform and working groups and representatives from each Local Government) who have responsibility for the day to day management of the PCP, operationalising the Healthy Communities Plan and setting directions for the work.


The partnership has Working Groups whose tasks are centred around the outcome areas as determined by Department of Human Servcies in the PCP Strategic Directions 2006-2009 and Implementation Plan for the Primary Care Partnerships.

The Upper Hume Primary Care Partnership supports a structure of engagement and participation of its members.  To this end, the UHPCP uses a Lead agency model and contracts from its members leadership and project management roles.  In line with this philosophy, the role of lead agencies, chairpersons (PCP general committee, each platform and working group) and administrative support is purchased from our members on an annual basis, in recognition of the workload required and to increase the capacity across our organizations and people.  The PCP is currently resourced by

  • Executive Officer - located at Upper Hume Community Health Service
  • Service Coordination Officer - shared position with Central Hume PCP - located at Albury Wodong Regional GP Network
  • Integrated Health Promotion - located with Upper Hume Community Health Service
  • Integrated Chronic Disease Management - located with Wodonga Regional Health Service.

We have focused also on age cohort groups Children and Families, Young People, Across the Years (issues focus) and Older People and we liaise with a number of other networks who interface the primary health sector.  These groups actively participate in planning, implementing, monitoring, reviewing and reporting and act as collegiate networks, planning platforms and and support with consultations, activities such as research, planning, events, information dissemination and participation.
