Our Mission
To enhance wellbeing and quality of life, reduce the prevalence, incidence and burden of illness/disability, meet the needs of hard to reach and vulnerable groups, and reduce health inequalities between population groups.
Our Key Objectives
Our key objectives are focussed around four areas of work:
- Partnership
- Integrated Health Promotion
- Service Coordination - including e-referral
- Integrated Chronic Disease Management
How we Operate
The UHPCP is structured around a General Committee. This committee comprises two levels of membership, full members and affiliate members that are signatories to the UHPCP Memorandum of Understanding. The General Committee is responsible for setting the direction of the PCP, decision-making, and ratification of the directions recommended by working groups and platforms.
The Upper Hume Primary Care Partnership supports a structure of engagement and participation of its members. To this end, the UHPCP uses a Lead agency model and contracts from its members leadership and project management roles. In line with this philosophy, the role of lead agencies, chairpersons (PCP general committee and working groups) and administrative support is purchased from our members in recognition of the workload required and to increase the capacity across our organizations and people. - Integrated Chronic Disease Management - located with Albury Wodonga Health - Wodonga Campus.
