Welcome to the Upper Hume Primary Care Partnership Website
Our partnership is building healthy communities in the Shires of Wodonga, Indigo and Towong and the Kiewa Valley of Alpine Shire.
Our Plan
Our Healthy Communties Plan 2006-2009 provides information and details of our work and an overview of the partnership and how we operate. The plan includes weblinks and background information to support people to build on their knowledge across various health and service delivery sectors.
See our Upper Hume PCP Annual Community Health Plan Report August 2008 which outlines what we've achieved over the first 2 years of the plan and reflects changes that have been made from our original document as work has developed or changed.
