What are PCPs doing to improve health outcomes for Victorians?

Each Primary Care Partnership produces a Community Health Plan. The plan outlines activities that are focused on building partnerships, integrated health promotion, service coordination and integrated chronic disease management. Each PCP is encouraged to tailor their plan to suit issues relevant to their geographic location (catchment). PCPs work together with their members to achieve mutually beneficial outcomes for the community.
