Upper Hume Community Participation Strategy - provides an overview for organisations on the areas of action and assessment for engagement and particiation of consumers/clients in their service.  This was developed in 2002 with the vision that the strategy will help us change the way we work with members of our community who may use our services from "doing for" to "doing with".

Upper Hume Health Profile       March 2005

This report provides quantitative health indicator information regarding the
communities located within the area served by the Upper Hume Primary Care Partnership (PCP), using the most recent data available.  

The report includes the provision of past data where it is available, and the provision of commentary regarding changes over time for many of those indicators.

The Health Profile 2004 consists of three major sections:

  • Demographic Profile
  • Health Profile
  • Social Factors

 This report has been prepared as one of a set of reports prepared for the four Primary Care Partnerships that make up the Department of Human Services Hume Region, North East Victoria, Australia
