Integrated Chronic Disease Management

Integerated Chronic Disease Management (ICDM) became a new area of work for the partnership with initial planning occuring in mid 2006 for inclusion into our Community Health Plan 2006-2009 .  At the same time Wodonga Regional Health Service was funded to deliver the Hospital Admission Risk Program (HARP). A HARP-ICDM Alliance was formed which covers both these areas of work. This group meets bi-monthly to share current data, trends and determine the strategies for the work in our area addressing chronic disease management.  

Work has commenced on ICDM priorities for 2009-2012 in the Upper Hume PCP.  See  ICDM 2009-2012

Key work occuring

  • Diabetes Self Management Project
  • Self managment mapping survey - completed by organisations - analysis occuring by Department of Human Services (July 2008)
  • Training has occured - regionally Health Coaching and Better Health Self Management.  Locally - Flinders model training. 
  • Hume region is planning further training - to run Motivational Interviewing.
