E-Referral - Connecting Care
There are now over 35 organisations and around 80 individual services / programs who are able to receive e-referral in Central and Upper Hume PCP's with the number growing each week (See attached Services Ready to receive list).
Note: There may be dual systems i.e. fax and e-referral operating in some organisations who are migrating their process to Connectingcare e-referral for a number of months so please be patient as organisations work through these changes. Also please encourage the use of Connectingcare to support service coordination practice within your organisation. If you have any questions about delivering training to other staff in your organisation or require support with e-referral please contact me during usual business hours.
Training: Train the Trainer sessions are conducted regularly or as requested.
