Service Access Models and Practice in Hume

The Service Access Resource Guide and the Service Access Tool Kit supports the development of efficient, effective and consistent consumer access to services. They also reflect individual organisation characteristices and local circumstances, including service demands.

This resouse supports organisations to review, select, implement and evaluate servcie access models. They outline the importance of service access models in supporting Servcie Coordination, identify various models and describe and compare benefits and challenges associated with each.  The accompanying toolkit has sample documents and templates to facilitate change management and support the review selection and evaluation of organisation service access systems.


Service Access Forum October 2008
A combined Service Access Forum was held at Rafferty's Benalla on the 23rd of October between Central and Upper Hume Primary Care Partnerships.  The quality of the presentations and the enthusiasm from all participants made for an excellent day.

The information gathered from the workshop at the end of the day, (see attached) is provided to assist agencies in their own activities around developing Service Access systems around service coordination and e-referral.  This and the evaluation data will also be used by the Central and Upper Hume PCP Service Coordination Steering Committees to improve uptake of best practice approaches around Service Access.
