The Upper Hume PCP members participate in numbers of projects to deliver health and wellbeing outcomes to our communities. These projects occur through funding attracted by the PCP and by various agencies and government departments.
The PCP provides an opportunity for information sharing and synergies of activities. Our Platforms and Working Groups provide openings for agencies to share information about projects and find ways to work together to enhance and support the outcomes of a project. A good example of this is the consortia of agencies and the Department of Human Services that are undertaking the Supporting Family Services – "Innovations" project occurring in Upper Hume.
The Upper Hume Primary Care Partnership Project Process:
The PCP attracts project funding through submissions. We identify an opportunity and then consult with members from specific groups or interested agencies to construct a proposal and write the funding submission. When successful we look to find a member agency who will Lead the project and roll it out in accordance with the funding agreement on behalf of the PCP.
Often projects involve a number of agencies working in small steering groups and participating in various aspects of the project. We look to work with the skills and experience of our agencies and build capacity and partnerships that enhance the outcomes for the agencies and the community.
