Completed Projects in Upper Hume

The Upper Hume PCP has now participated in the success of a number of projects across the shires of Towong and Indigo and the City of Wodonga.

May  2008

Partnerships in Safer Sex and Testing (PSTT)

PSTT Final PSTT report May 2008

PSTT was a 10 month project funded by Family Planning Victoria and led by Upper Hume Communtiy Health Service.  Significant need identified for improvement in education and service levels.  

Feb 2008

Friends for Life  - part of the Tackling Mental Health in Drought work in our area.   Friends Evaluation 2007  This innitiave trained over 40 teachers and welfare workers in our area to deliver the Friends program into schools. The aim to build the mental health and resilience of young people in surviving the tough and stressful time of the drought.

General Practice Service Coordination project
This service corordination project explored referral pathways and care coordination between General Practice and public and private health services.

Falls Prevention

Summary Report on Foothold on Safety March 2007.  

Community Falls Prevention project: increased information and training being provided to professionals, carers and interested community groups to address safety in the home and community. Collaboration of agencies: Upper Hume Community Health Service, City of Wodonga , and Wodonga Regional Health Service. Walktober Small grant of $1000 provided support to a number of local activities celebrating Walktober.

2006 Dec

Walking Pathways $10,000 1 year project funded through Department for Victorian Communties. The project is developing information packs and logbooks to increase participation in walking and to support walking initiatives. UHPCP QIPPS record

  Fad Diets Won't work report  and Fad DietsPowerpoint 

Research into the influences on nutrition and lifestyle
choices of young people. Methods included professional forum and 'Community conversations' involving young people, schools, parents and community providers. Lead: Wodonga Regional Health Service and the Healthy Living Action Team of 
health promotors in our area. 

2006

Rural Health Week Activies held across the area promoting health and wellbeing and social connectness. 1Groups of La trobe Unversity second year Bachelor of Nursing students worked with our members to provide education sessions at ten schools.

2005

Secure Electronic Referral Linkages in the Upper Hume PCP
June 2005

$10,000.  Service coordination with General Practice Small Grant from DHS. Aim: for GPs and their practice managers/nurses to utilise more fully the Victorian Statewide Referral Form for the purposes of referral and service coordination with other service providers using secure electronic referral via email.

Services for Men in Upper Hume Evaluation
February 2006

An evaluation of the services provided specifically for men in the Upper Hume catchment. 17 men's programs responded via a questionnaire and overview of the programs they are running. The Men's Community Reference Group worked with David Bacash a La Trobe University Bachelor of Social work to undertake this evaluation

Snapshot of Community Participation in Upper Hume the study provides a snapshot of the methods of engagement and aprticipation that agencies use and attempts to identify the enablers, barriers, gaps and successful strategies and pariticipatory methods.

Health Promotion Capacity Review 2004 Project to review the capacity of our organisations and workforce in health promotion since the audit of 2002.

Good Beginnings for young children and
families – feasibility study

Research into the use of the Parental Evaluation Development Status questionnaire that identifies children’s developmental progress. Work focused on creating a coordinated service framework using this process.

To identify health promotion initiatives occurring within the Upper Hume region involving both UHPCP member and non-member agencies.

Rural Counselling Project -
Improving access and response to
counselling services in the Upper Hume PCP.

To develop a model for the provision of coordinated counselling service within the UHPCP catchment.
